Voluntary Life and AD&D Insurance Plans Employees may purchase the following voluntary life and AD&D insurance coverage: • Employee Life Insurance in $10,000 increments, up to a maximum of $50,000 and not to exceed the guarantee issue amount of $100,000. Employees can elect up to $100,000 of coverage without having to provide Evidence of Insurability (EOI). Any employee can choose up to $500,000 of coverage, subject to EOI. • Spouse Life Insurance in $5,000 increments, up to a maximum of $20,000 and not to exceed the guarantee issue amount of $50,000. Employees can elect up to $50,000 of coverage without having to provide EOI. Any spouse can choose up to $250,000 of coverage, subject to EOI. • Child Life Insurance of up to $20,000 per child. • Voluntary AD&D Coverage equal to one to six times annual base salary for employees and/or their families. Coastal Housing Partnership UPDATED: Cottage Health is a founding member of the Coastal Housing Partnership, a non-profit network that offers services to employees when purchasing a home, refinancing a mortgage or leasing a new apartment. When someone takes advantage of all the partnership has to offer, the cost savings can total as much as $20,000 with discounts on closing costs, commissions and other fees when purchasing a home. In the past year, 127 employees used the Coastal Housing Partnership benefit for total savings of $327,250. One physical therapist and her husband rented in the community for many years, thinking they would never be able to afford a home. After becoming a CH employee, she attended several of the free home buying workshops offered by the Coastal Housing Partnership. She followed the program’s advice and accessed its resources. She engaged a realtor, lender and an escrow company through Coastal Housing’s preferred network service partners. This saved her approximately $15,000, and she also utilized the Cottage Mortgage Assistance program which provided her with the additional funds needed towards her down payment. Without access to these discounts and resources, this employee may have moved from Santa Barbara and gone to another employer. Onsite Employee Discount Pharmacy Prescription drug costs generally account for a substantial portion of most people’s healthcare costs. However, employees and their families enrolled in a Cottage medical plan have the advantage of prescription drugs offered at discounted rates along with the convenience of an employee pharmacy. For a 30-day supply of generic drugs, employees pay either 30% of the discounted cost or a $5 minimum. For brand-name drugs, employees pay either the discounted cost or a $10 minimum.
